The information provided below briefly explains why Park Lane Medical Centre collects information about you, how we keep it safe and confidential and how that information may be used.

Why we collect information about you

Health care professionals who provide you with care are required by law to maintain records about your health and any treatments or care you have received within any NHS organisation.  We collect and hold data for the sole purpose of providing healthcare services to our patients.

Details we collect about you

As healthcare professionals who provide you with care we maintain records about your health and any treatment you may have received.  Records which we may hold about you include:

  • Your name, address, date of birth, next of kin & emergency contacts
  • Your home or mobile telephone number and/or email address
  • Any contact the practice has had with you such as appointments, clinic visits, immunisations etc
  • Notes and reports about your heath, treatment or care
  • Results of investigations such as laboratory tests, x-rays etc
  • Relevant information from other health professionals

How we keep your information confidential and safe

All GP NHS health records are kept electronically.  Our GP database is hosted by EMIS Healthcare Ltd, who is acting as Data Processor, and all information stored on their secure servers is protected by appropriate security and access is restricted to authorised personnel.

We maintain our duty of care to you always and will only pass on information about you if others involved in your care have a genuine need for it.

How we use information about you

Accessing your own medical information

You have the right to access your own GP record.  Details of how you can do this are available on our practice website you can also sign up to have secure online access to your electronic medical record by completing the form in the New Patient Registration Pack.


The General Data Protection Regulations 2018 requires organisations to register notification with the Information Commissioner to describe the purpose for which they process personal and sensitive information.  We are registered as a data controller and our registration can be viewed online in the public register at:
