Change of address and telephone numbers

It is imperative that we are kept up-to-date with any change in your contact number – both landline and mobile. Please ensure you inform us of any changes ASAP.

Please inform reception staff if you change your address within our area. You will need to complete a change of address notification form. (You may also need to bring proof of your change of address – such as a Utility bill in your name for your new address).

If you move out of the area you will need to register with a local practice near to your new address as soon as possible.
